How under chin fat and platysma anatomy sit in relation to treatment for a double chin

What is Under-Chin Sculpting?

Under-chin sculpting is a consultation-led approach to assessing and discussing concerns relating to fullness beneath the jaw and chin. Often described as a double chin, this area can be influenced by under-chin fat, skin laxity, facial structure and the angle between the chin and neck. Under-chin sculpting is a non-surgical option to addressing a double chin under the jaw.

What Causes A Double Chin?

Under-chin fullness, also known as a ‘double chin’, can be due to a variety of factors, including:

  • Excessive under-chin fat (most common)
  • A weak under-projected chin reduces the angle between the chin and neck, making it appear as if a double chin exists; this is known as a ‘pseudo double-chin’
  • Genetics
  • Overly loose neck skin due to loss of collagen as a part of the ageing process
  • Weight gain
  • Overall lower-face structure and anatomy

For some people, under-chin fat is the main concern. Under chin fat (pre-platysmal fat) sits above a large thin sheet of muscle called the platysma (pre-platysmal fat). This fat tends to stubbornly remain despite our best efforts to remove it with diet and exercise. This fat sits close to the skin, making it, in most cases, amenable to treatment.

In others, the under-chin area may be influenced more by skin laxity, chin position or a combination of factors. That’s why we start with a consultation – it allows us to understand what may be most relevant for you and then to determine next steps.

Why People Seek Double Chin Treatment

People often seek double chin treatment because fullness under the chin can affect how the lower face and neck look in profile and at rest. In consultation, common discussions may include:

  1. Wanting more definition through the chin, jaw and neck area
  2. Contouring the appearance of the chin, jaw and neck area, refining the definition of your lower face. Further definition can be provided with a Chin Augmentation.
  3. Understanding whether the concern is fat-related, skin-related or structural
  4. Minimising the appearance of a double chin or under-chin fullness that has not shifted despite weight loss or lifestyle changes
  5. Lower-face heaviness or concerns around fat cells
  6. Addressing the appearance of a cobblestone chin or chin creases
  7. Non-surgical and minimally invasive cosmetic procedure
  8. Tailored treatment plans overseen by experienced cosmetic doctors

What To Expect in Your Under-Chin Consultation

At Contour Clinics, our experienced cosmetic doctors and nurses provide comprehensive consultations to carefully evaluate the specific anatomical reasons for your under-chin fullness. • Medical history and suitability screening, including relevant medications and health factors
  • Anatomy-led assessment of your chin, under-chin and neck to evaluate structure, symmetry, skin quality, and where relevant, movement patterns
  • Standardised clinical photography and, where appropriate, Vectra 3D imaging to map proportions and establish a baseline
  • A clinician-led discussion of options that may be suitable for you, including limitations, trade-offs and realistic expectations
  • Clear explanation of risks, aftercare and recovery considerations, tailored to your circumstances
  • A written plan and next steps you can review in your own time, with space to ask questions and decide without pressure

Double Chin Treatment Approaches That May Be Considered

Because a double chin or under-chin fullness can be influenced by more than one factor, different approaches may be discussed. The most suitable one for you depends on whether your concern relates mainly to fat, skin laxity, chin projection or overall lower-face support. These are the approaches that may be explored with your Contour Clinics clinician:

Morpheus8 Radio-Frequency Skin Needling
This may be considered where mild to moderate skin laxity, crepey texture or reduced support through the neck area are part of the concern. It is generally considered when skin quality is contributing more than volume or structure alone.

CoolSculpting & Fat Freezing
This may be discussed where localised under-chin fat appears to be the main contributing factor. Your clinician will assess whether this is appropriate for your anatomy and whether the concern appears fat-related rather than structural or skin-related.

PDO Mono Threads
PDO mono threads may be explored where skin support and mild laxity are part of the concern, particularly in cases where the area beneath the chin and along the upper neck looks less supported over time. This approach is usually considered as part of a broader skin-quality and support discussion.

Skin Tightening

If reduced firmness or skin laxity is contributing to fullness under the chin, your clinician may discuss skin tightening approaches as part of a broader plan. This can be relevant where the concern is not only fat, but also how the skin sits over the area.

All discussions about your concerns are guided by your anatomy, goals and suitability, with likely benefits, limitations, risks and realistic expectations explained clearly during consultation. The ideal treatment option depends on your treatment goals and how you feel about downtime, swelling and recovery.

Frequently Asked Questions

How do fat reducing treatments under chin work?

Cosmetic treatments for under-chin fullness generally aim to reduce the appearance of the area beneath the chin by addressing subcutaneous fat. Certain non-surgical cosmetic options are specifically designed to reduce small amounts of fat beneath the skin over time.

Your Contour Clinics Cosmetic Doctor will discuss how under-chin fat reduction treatments work in your complimentary consultation.

Are fat reduction treatments safe?

All medical procedures carry risk. The risks as they relate to you will be discussed during your complimentary consultation with one of our experienced cosmetic doctors or nurses.

Do fat reduction treatments hurt?

At Contour Clinics, we ensure your comfort at each step of the process. Using a combination of numbing cream and local anaesthetic, we ensure discomfort is kept to an absolute minimum.

Fat reduction treatments can be incredibly painful if a local anaesthetic is not used. We believe a local anaesthetic is mandatory when treating this area.

Once the local anaesthetic wears off in 1-2 hours, you may feel a slight discomfort or ache. This is uncommon and can be helped with paracetamol and icing to the treatment area.

Do fat reduction treatments get rid of cellulite?

No. Unfortunately, cellulite does not respond to targeted fat reduction treatments. There are many other cellulite treatment options our team can discuss with you at a complimentary consultation. You can read more on our Cellulite Treatment page.

What areas of the body can you get a consultation for fat reduction?

Our cosmetic doctors perform complimentary consultations regarding the reduction of fat pockets in areas of the body, including:

  • Under chin
  • Jowl fat
  • Bra fat
  • Above-knee fat
  • Small pockets around the abdomen

Currently, consultations are limited to small areas of fat accumulation. We don’t recommend treating large areas with non-surgical methods.

Who is not a candidate for non-surgical fat reduction treatments?

You are unable to have non-surgical fat reduction treatments if you are:

  • Are pregnant
  • Are breastfeeding
  • Have specific allergies
  • Have an active infection/skin condition over the area to be treated

Are non-surgical fat reduction treatments permanent?

Any fat cells reduced as a result of treatment are likely to be removed permanently. There are no guarantees, and your doctor will provide advice around expectations as they may relate to your unique circumstances.

Are non-surgical fat reducing treatment results instant?

Results vary person to person. Consult with your cosmetic doctor to understand time frames as they relate to your unique circumstances.

How does under chin Coolsculpting work?

Under chin CoolSculpting works by freezing excessive under-chin tissues to minus 11 degrees Celsius. We know that at these temperatures, the skin/blood vessels/nerves are unlikely to sustain damage.

Fat cells, however, cannot stand these temperatures for a prolonged period of time and undergo a self-destruction process called ‘programmed cell death’. The fat cells’ membranes break apart and release their contents, which are metabolised by the body.

The destroyed fat cells are then cleared away by the body’s immune system over 6-12 weeks.

Is Coolsculpting under the chin safe?

This is a non-invasive technology that has been approved by regulatory bodies and peer-reviewed literature as having an acceptable safety and effectiveness profile.

What are the potential side effects of Coolsculpting?

The most common include swelling and bruising. These are mild and short-lived, and tenderness in the treated site.

When can I see the results of Coolsculpting?

You can notice results 4-6 weeks post treatment, with full results seen 3 months post treatment.

How many under chin Coolsculpting sessions will I need?

Typically, a treatment protocol consists of 1-3 sessions spaced 1 month apart.

How do I choose the best option for me?

The most suitable option depends on what is contributing to fullness beneath the chin and which approach is appropriate for your anatomy, goals and preferences. During your consultation, your clinician will assess whether the concern appears more related to under-chin fat, skin laxity, chin projection or a combination of factors.

Discussion may include considerations such as:

  • The likely recovery experience and whether swelling or downtime may be part of the treatment pathway
  • Whether a single treatment or a series may be more appropriate
  • Whether the approach is non-invasive, minimally invasive or more focused on skin support
  • How the treatment plan aligns with your preferences, lifestyle and budget

Your clinician will explain the likely benefits, limitations, risks and realistic expectations of any suitable option, so you can decide with a clear understanding of what may be appropriate for you.
