About the area What is a tear trough and how does it affect my appearance?

The tear trough is the region of hollowing between the lower eyelid and the cheeks.

Problems in this area of the face can leave you with dark circles, under-eye hollowing and a tired appearance, even if you’ve had adequate sleep.

This hollowing can be accentuated by ageing, weight loss, fatigue and occasionally sinus conditions.

Hollow tear troughs are usually related to genetics, or can be due to lack of lower eyelid support caused by mid-cheek volume loss.

Assessment of Your Under-Eye Area

Treatment of the under-eye region varies from person to person and often requires an individualised treatment plan.

This is determined by a careful examination of your unique under-eye anatomy, and understanding your individual aesthetic concerns.

Assessment of the tear trough area requires a thorough inspection of five distinct considerations:

  • Depth of tear trough 
    • Hollow tear troughs can be treated with a thin dermal filler that has been specifically designed for this area.
    • Moderate-to-severe tear troughs may require a staged approach; 1ml of filler per session, performed every 2-4 weeks, until the desired result is achieved.
  • Degree of mid-face volume deficit
    • The tear trough region requires support from the cheek beneath it. Flattened cheeks can cause the appearance of a hollow tear trough due to the lack of support provided to the above under-eye area.
    • Correcting mid-face volume deficit can significantly improve the under-eye area.
  • Hollowing of the palpebromalar groove
    • This is the groove on the outer aspect of the tear trough. Filling this area can smooth the transition between outer eye and cheek, while providing support to the inner under-eye region.
  • Eye bags 
    • Under-eye fat bags can become more prominent with age. This can be due to loss of surrounding camouflaging tissue, laxity of overlying skin, or stretching of under-eye compartments containing orbital fat pads.

What can I expect during my tear trough filler treatment?

After your complimentary in-clinic consultation, professional assessment and collaborative planning, prescription-grade numbing cream will be applied to your under-eye area.

The area is then cleaned, then based on your assessment filler is placed expertly along the orbital rim to improve under eye volume and decrease under eye circles.

Where is tear trough filler placed?

This depends on individual differences in unde-eye hollowing.

Some patients require filler placed around the bony rim of the eye socket. This will bolster muscular attachments, revolumise lost fat compartments, and support around-eye ligaments to improve the under-eye aesthetic.

Some need cannula placement (blunt needle) closer to the inner eye to improve under-eye dark circles, and restore lost volume.

How much under-eye filler do I need?

This depends on the individual.

Generally, we place 0.5ml each side initially. Then we will review your result 2 weeks post treatment to assess whether further treatments are required.

Most patients need only 0.5ml per side for best results.

Can tear trough filler go wrong?

There is risk involved with any medical procedure. However, the risk of tear trough filler complications is very rare.

In our hands, bruising is unlikely and only occurs for 1 in 5 patients. This is usually short lived and easily covered by makeup.

Occasionally, tear trough filler can temporarily increase under-eye puffiness. This is usually due to filler placed in patients that aren’t suitable candidates for the treatment, such as those with large under-eye bags.

More serious complications are incredibly rare and will be explained in detail during your complimentary consultation.

Can tear trough filler be dissolved?

Yes. In the case of a poor cosmetic result, or in the unlikely event of a rare complication, tear trough filler can be reversed immediately.

Your under-eye area will return to its original pre-treatment state within 1 week of reversal.

Does tear trough filler move?

Though rare, this can occur if you have under-eye treatments with an inexperienced injector. Filler can move if they are placed above the muscle surrounding the eye. Contraction of this muscle over time moves the filler towards the midline.

Which under-eye filler is best?

Does tear trough fillers get rid of dark circles?

Often tear trough fillers can improve the appearance of dark under-eye circles.

Fillers increase the distance between the skin and underlying veins, and thereby decrease the visibility of the dark hue given off by under-eye veins.

Some darkness under eyes is caused by pigmentation.

This will not be improved by fillers; our specialist laser treatments for hyperpigmentation are best for treating stubborn under-eye discolouration.

Can tear trough filler cause dark circles?

When expertly placed, this is not a concern.

If placed too superficially, the under-eye area can develop a dark blueish tinge. This is always due to poor injection technique.

If this has happened to you at another clinic, we can reverse the effect and re-inject filler into the appropriate plane to eliminate this issue.

Can tear trough filler get rid of wrinkles?

This depends very much on the severity of wrinkles.

You may notice a decrease in fine lines after volumisation of  the under-eye area with filler. However, this often requires a combination treatment approach including laser, and anti-wrinkle injections.

Do under-eye fillers get rid of bags?

Unfortunately, those with large eye bags are not suitable for under-eye filler.

This issue is best treated with our TempSure skin tightening treatment. In severe cases, surgery may be the most effective option.

How long do tear trough fillers last?

These fillers last 9-18 months depending on individual characteristics.

Does tear trough filler hurt?

You will feel minimal pain during this procedure.

There is some discomfort when the cannula is positioned but this is short lived. Most people describe an uncomfortable sensation rather than pain. Many of our patients report anti-wrinkle injections as more painful.

Do under-eye fillers cause bruising?

Only 1 in 10 patients bruise with under-eye filler. This varies from a pinpoint bruise to under-eye area darkness. This resolves within 3-5 days and is easily covered by makeup.

How long is under-eye swelling after the procedure?

Temporary under-eye swelling is different for every patient. Most patients leave our clinic without a hint of redness or bruising.

Some patients have mild bruising and minor swelling lasting 3-5 days. This can easily be covered by makeup.

How long for tear trough fillers to settle?

Complete integration within the tissues surrounding the eye takes 2 weeks. We recommend a complimentary review appointment at this stage to check your progress.

How to massage under-eye filler?

We provide under-eye massages after every tear trough treatment.

Your treating doctor will explain exactly how to massage the area. However, it’s usually unnecessary for you to do this yourself, as we massage out any lumps before you leave the clinic.
