Pitted skin from acne discussed in a consultation with a cosmetic doctor about how to treat acne scars with volumisation

Understanding Acne Scarring

Acne scarring can occur when the skin’s collagen fibres are produced and arranged in an irregular way during the healing process. This may result in pitting or changes to the contour of the skin.

There are several types of acne scarring, and the approach discussed in consultation will depend on the type and extent of the scarring.

  • Atrophic scars – shallow depressions in the skin, sometimes associated with severe acne but also possible with other forms.

    • Ice-pick scars: narrow, deep indentations

    • Rolling scars: broader depressions with a sloping edge

    • Boxcar scars: broader depressions with more defined edges – these may sometimes be suitable for treatments designed to restore volume beneath the skin.

  • Hypertrophic or keloid scars – thicker, raised scars caused by excess scar tissue formation. These are not typically addressed with volume restoration treatments, as they do not involve a loss of skin depth.

For some types of pitted acne scarring, your clinician may discuss options that help support the contour of the skin in selected areas. Your clinician will assess whether this is appropriate for you and discuss alternative options.

How Volume Restoration Treatments May Work

For selected types of pitted acne scarring, one option that may be discussed is an approach aimed at supporting the contour beneath the area of concern. This may help soften the appearance of uneven texture in suitable cases.

Suitability, product choice and placement technique will depend on your skin, the type of scarring and your treatment goals. Not all acne scars are suited to this approach, which is why careful assessment comes first.

Visible changes vary between individuals, and your clinician will discuss what can reasonably be expected in your case.

The Consultation Process

During Your Appointment
A qualified Cosmetic Doctor or Senior Cosmetic Nurse will assess your skin and discuss the type of acne scarring present, your goals, and which options may be appropriate to discuss. If treatment is considered suitable, the area will be cleaned and a topical anaesthetic may be applied. In some cases, a local anaesthetic may be recommended.

After Your Appointment
For the first 24 hours, you may be advised to avoid alcohol, makeup, pressure on the area, or heat exposure such as saunas and hot yoga. Your clinician will provide specific aftercare instructions.

It is generally recommended to avoid alcohol and certain anti-inflammatory medications in the lead-up to treatment, as these may increase the likelihood of bruising. If you are taking medication prescribed by your GP, do not stop without medical advice.

In the Following Days
You may notice some tenderness or mild swelling, which usually subsides within a few days. Your clinic may offer a review appointment after 1–2 weeks to assess the outcome and discuss whether any further treatment is appropriate.

Other Approaches That May Be Discussed For Acne Scar Treatment

Acne scar treatment is not one-size-fits-all. Depending on the type of scarring, skin texture and overall skin quality, your clinician may discuss different approaches in consultation.

Where relevant, discussion may include:

  • Approaches for pitted or atrophic scars
  • Options that focus more on skin texture and surface irregularity
  • Whether contour support may be appropriate in selected cases
  • Whether a staged plan or combination approach may be more appropriate than one treatment alone
